The Genetics of Longevity: What Actually Replicates
Exceptional longevity runs partly in the genes, but the solid signals are few, and most don't translate into anything you can buy or do.

FOXO3A is the most reproducibly longevity-associated human gene after APOE. The rs2802292 G allele was tied to exceptional longevity in Japanese-American men (OR 2.75 for minor-allele homozygotes) [1], and replicated across German [2], Italian [3], Danish [4], and Chinese [5] cohorts, with a pooled GWAS confirming only APOE and FOXO3 survive genome-wide (OR ~1.17) [6]. The longevity allele is functional, it creates a heat-shock-factor binding site that raises FOXO3 expression under stress [7].
The gene sits on the insulin/IGF-1 (IIS) axis, the most conserved aging pathway known. Humans with lifelong growth-hormone-receptor loss of function (Laron syndrome) develop no cancer and no diabetes over 17 years [8]; a GH-receptor exon-3 deletion tracks with ~10 extra years of life [9]; and partial IGF-1-receptor loss-of-function mutations are over-represented in centenarians [10]. IGF-1 itself follows a U-shaped mortality curve, both low and high are harmful [11]. Barzilai argues this U-curve is female-specific and that centenarians carry a "portfolio" of protective variants offsetting ordinary disease risk [12].
APOE is the other major axis, and it cuts both ways. APOE4 raises Alzheimer's risk dose-dependently (ε3/ε4 OR ~3.2; ε4/ε4 OR ~14.9) [13, 14], and ε4/ε4 homozygosity now looks like a near-fully-penetrant genetic form of the disease, with biomarkers abnormal from age ~55 [15]. The ε2 allele does the opposite, ~40% lower AD odds [13], ~13% lower coronary risk in the common ε2/ε3 state [16], and better survival even after adjusting for Alzheimer's [17].
PCSK9 is the cleanest "genetics → drug" success story in the vertical. Gain-of-function mutations cause familial hypercholesterolemia [18]; loss-of-function mutations lower lifelong LDL and cut coronary disease up to ~88% [19]; and two outcome trials confirmed that drugging the target works (evolocumab MACE HR 0.85 [20]; alirocumab HR 0.85 [21]).
But "longevity gene" rarely means "actionable." The cautionary case is CETP: a variant is the single strongest longevity signal in the Ashkenazi centenarian cohort [22], yet three large CETP-inhibitor trials failed, torcetrapib caused harm [23], dalcetrapib [24] and evacetrapib [25] were flatly null despite huge HDL increases. Raising HDL pharmacologically does not reproduce the genetics.
